The <strong>scaphoid<\/strong> is located in the outer side of the wrist, between the thumb and the radius. It is called that way because it is thought to look like a boat. It also looks like a large cashew nut. The most common mechanism is a <strong>fall onto the outstretched hand<\/strong>. It can also occur during sporting activities or as a result of a road traffic accident.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<h2 class=\"wp-block-heading has-medium-font-size\" id=\"\u03c3\u03c5\u03bc\u03c0\u03c4\u03ce\u03bc\u03b1\u03c4\u03b1\">Symptoms &amp; clinical picture<\/h2>\n\n\n\n<p>The symptoms of a fractured scaphoid bone may include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Pain with gripping<\/li>\n\n\n\n<li>Swelling and bruising of the wrist<\/li>\n\n\n\n<li>Pain in the anatomical snuffbox&nbsp;<\/li>\n\n\n\n<li>Pain on the radial styloid<\/li>\n\n\n\n<li>Pain upon axial loading of the thumb<\/li>\n<\/ul>\n\n\n\n<p class=\"translation-block\">Symptoms may be very subtle initially. It is not uncommon for a scaphoid fracture to go undiagnosed for many months after the injury, or manifest itself many years later, after the development of wrist arthritis.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<h2 class=\"wp-block-heading has-medium-font-size\" id=\"\u03b4\u03b9\u03ac\u03b3\u03bd\u03c9\u03c3\u03b7\">Diagnosis - why is a high index of suspicion needed?<\/h2>\n\n\n\n<p class=\"translation-block\">The doctor will see you, take a detailed history and perform a physical exam, with care not to cause and further pain or discomfort. If a scaphoid fracture is suspected, x-rays of the area will be ordered (usually 3-4 high quality <strong>scaphoid views<\/strong>). Early on your fracture may not be visible. In case of a strong suspicion, even in the setting of normal initial x-rays, you may be placed in a cast and get a follow up in <strong>10-14 days<\/strong>. At that point, a scaphoid fracture will be apparent on x-rays. Alternatively, your doctor may order an CT scan, or an MRI.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-uagb-blockquote uagb-block-f25eab08 uagb-blockquote__skin-border uagb-blockquote__stack-img-none\"><blockquote class=\"uagb-blockquote\"><div class=\"uagb-blockquote__content translation-block\"><em><strong>High index of suspicion!<br><\/strong><\/em><br><strong>Exam:<\/strong>\u00a0snuffbox tenderness.\u00a0<br><strong>Xrays:<\/strong>\u00a0often initially normal - rexray in 10-14 days upon strong suspicion.\u00a0<br><strong>MRI \/ CT:<\/strong>\u00a0if xrays are negative, or in controversial cases.<\/div><footer><div class=\"uagb-blockquote__author-wrap uagb-blockquote__author-at-left\"><\/div><\/footer><\/blockquote><\/div>\n\n\n\n<p><\/p>\n\n\n\n<h2 class=\"wp-block-heading has-medium-font-size\" id=\"\u03b8\u03b5\u03c1\u03b1\u03c0\u03b5\u03af\u03b1\">Treatment<\/h2>\n\n\n\n<p class=\"translation-block\">Each scaphoid fracture is different. Treatment may depend on time elapsed since the injury, degree of displacement, location of the fracture (distal, waist, proximal), prior treatment failure, etc. The scaphoid bone is notorious for not healing well due to its <strong>precarious blood supply<\/strong>. Casting is generally a safe and effective treatment for non-displaced fractures of the waist of the scaphoid. Casting time, however, may vary from 6 to 12 weeks. Your doctor will follow you with serial x-rays until bone healing is achieved clinically and radiologically.<\/p>\n\n\n\n<p class=\"translation-block\">Surgery is indicated for <strong>displaced fractures<\/strong>, fractures of the <strong>proximal pole<\/strong> of the scaphoid, or fractures that have <strong>failed to heal<\/strong>, or upon <strong>patient request<\/strong> if they want to get back to action as soon as possible (professional athletes or manual laborers). Surgery may be open or percutaneous based on the situation. This condition is painful and will compromise your whole wrist. Surgery of scaphoid nonunion involves open surgical fixation with bone graft, taken locally or from your pelvis. Grafts can be either vascularized or non-vascularized, with current studies showing no significant difference. <\/p>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-28f84493 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"border-width:1px;border-top-left-radius:10px;border-top-right-radius:10px;border-bottom-left-radius:10px;border-bottom-right-radius:10px;padding-top:var(--wp--preset--spacing--60);padding-right:var(--wp--preset--spacing--60);padding-bottom:var(--wp--preset--spacing--60);padding-left:var(--wp--preset--spacing--60)\">\n<h3 class=\"wp-block-heading\" id=\"\u03b8\u03b5\u03c1\u03b1\u03c0\u03b5\u03af\u03b1-\u03c3\u03c5\u03bd\u03c4\u03b7\u03c1\u03b7\u03c4\u03b9\u03ba\u03ae-vs-\u03b5\u03c0\u03b5\u03bc\u03b2\u03b1\u03c4\u03b9\u03ba\u03ae\"><strong>Treatment: Non-operative vs. Surgery<\/strong><\/h3>\n\n\n\n<h4 class=\"wp-block-heading translation-block\" id=\"\ud83d\udccc-\u03c3\u03c5\u03bd\u03c4\u03b7\u03c1\u03b7\u03c4\u03b9\u03ba\u03ae-non-surgical\">\ud83d\udccc\u00a0<strong>Conservative treatment <\/strong><\/h4>\n\n\n\n<p class=\"translation-block\">Stable &amp; undisplaced fractures:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"translation-block\"><strong>Splint\/cast<\/strong> including the thumb for at least 6-12 weeks.<\/li>\n\n\n\n<li>Follow up with serial xrays.&nbsp;<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\" id=\"\ud83d\udfe0-\u03c7\u03b5\u03b9\u03c1\u03bf\u03c5\u03c1\u03b3\u03b9\u03ba\u03ae-\u03b1\u03bd\u03c4\u03b9\u03bc\u03b5\u03c4\u03ce\u03c0\u03b9\u03c3\u03b7\">\ud83d\udfe0&nbsp;<strong>Surgical treatment<\/strong><\/h4>\n\n\n\n<p class=\"translation-block\">For <strong>displaced fractures<\/strong>, unstable patterns, or if there is a <strong>high risk of nonunion<\/strong>:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"translation-block\"><strong>Surgical fixation<\/strong> with a headless compression screw +\/- bone graft.<\/li>\n\n\n\n<li class=\"translation-block\"><strong><a href=\"https:\/\/drgpanagopoulos.com\/en\/arthroskopisi-karpou\/\" target=\"_blank\" data-type=\"page\" data-id=\"6370\" rel=\"noreferrer noopener\">Arthroscopy<\/a><\/strong> may be used as an aid.<\/li>\n<\/ul>\n<\/div>\n<\/div>\n\n\n\n<p><\/p>\n\n\n\n<p class=\"translation-block\">It is imperative that you quit <strong>smoking<\/strong>, as this compromises any chance of healing by at least 50%. Often times, a scaphoid fracture may coexist with associated injuries, such as distal radius fractures or perilunate dislocations, that will necessitate surgical treatment in the same setting.<\/p>\n\n\n\n<p class=\"translation-block\"><a href=\"https:\/\/drgpanagopoulos.com\/en\/orthopaidikos-xeirourgos\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Dr Panagopoulos<\/strong><\/a> has extensive experience in the treatment of wrist fractures, and will discuss with you all options during your visit in our office.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<div class=\"wp-block-uagb-blockquote uagb-block-a172743d uagb-blockquote__skin-border uagb-blockquote__stack-img-none\"><blockquote class=\"uagb-blockquote\"><div class=\"uagb-blockquote__content translation-block\">A <strong>scaphoid fracture<\/strong> is not a simple \"sprain\" \u2013 it is a serious injury with <strong>predictable complications<\/strong> if not recognized and treated promptly and correctly.
